An engine tick, to be exact. Hoping this is just the timing chain telling me it's tired or some other relatively non-destructive issue. Looked for 4.2L engine noise videos on YouTube and didn't find anything similar.

It produced this sound before and after a drive, so being warmed up or not makes no real difference.

Tell you the truth, I couldn't hear anything that I would be concerned about on a 02 vehicle.
 
That noise in your video sounds like the a/c compressor is cycling, on and off. Turn the defroster off or a/c off or just shut down the entire heating-a/c system and see if noise goes away.
 
All I heard was an odd click here and there, as @movietvet said, like an A/C clutch cycling. Try pulling the A/C clutch relay. To see if it's something belt driven, take the belt off and start it. No noise, then it's something belt driven. Would also be a good time to check stuff like the water pump, PS pump, pulleys, etc.

And I also remembered something specifically for 02. They did have a ticking issue with the fan clutch. GM issued a PCM update for that as well as un updated fan clutch but I would imagine it would have been replaced by now. The update might fix it.

Guess I should have specified which noise I was referring to. My bad. I meant the persistent noise heard throughout the video that in retrospect is better described as a "rattle" than a "tick". It's a bit more noticeable IRL, phone camera doesn't do a great job picking it up.

The sound I'm referring to fades when I pan the camera left, and gets a bit louder when I pan it right.

Doesn't sound like there's a serious problem, though now I'm noticing the other "tick" or "click" that I think everyone else latched on to. I was so focused on the engine noise I didn't even notice it.

You mean like a sewing machine noise? Maybe the timing chain? Was it ever quiet before? Kinda sounds like mine actually.

When I first road tested my girl's 2002 TB, I posted on here in December of 2022 and asked about the engine noise being normal. I was told my a member, I forget who, that the engine sewing machine sound was normal. IMO, that is a normal sound for the 4.2 because her TB runs perfect.

Yeah, something like that. Listening to videos of other engines timing chain noise is the closest sound I've found. I swear it was quieter in the past but maybe I'm just being paranoid or noticing things I haven't paid much attention to before now that I'm really going through the whole vehicle.

Anyway, sounds like I don't need to concern myself with this right now.
 
Mine makes a clicking sound when it's warming up. It's the heat shield and the exhaust manifold coming up to operating temperature. It stops when it's warmed up.

I know you said it was there all the time so it's probably something else.
