My 07 Trailblazer is leaking fluid at the very back closer to the driver side just behind the rear bumper. I suspect it's washer fluid going to the rear window. Does anyone have a layout of the washer lines ?

This is a "Generic" Diagram to give you an idea how GM generally routes the Windshield Washer Fluid Line along the Driver's Side of the underbody to the rear of the SUV:

After Topping Off the Windshield Washer Reservoir... Lay Fresh Card Board under the SUV overnight to see the Drip Pattern in the morning after to help get an idea of where to isolate the leak.

Any idea what part #27 is called?

THAT particular Number will probably be for a Different Model SUV. However... Visit THIS Link for the 2007 Chevrolet Trailblazers and poke around at this particular 'gmpartsdirect, com' site and see if you can get a Hit that is Closer to The Mark on what you are after...

#27 looks like a grommet or a seal.

That's exactly where it's dripping from, a small rubber hockey puck shaped grommet near the spare tire. I might try to block off the line going to the back window, just not sure where the best location would be to do it.

Right at the pump. I don't remember which one it is. Pull the hose and use a vacuum plug.
