Rear HVAC Issues Codes present

Hey Guys,

I have the following codes coming from my Rear HVAC codes on my 2004 TrailBlazer EXT.

B0428
Temperature Control 3 Circuit Malfunction

B0145
Air Flow Position Command 3 Circuit

My question is where can i find these 2 actuators and what par numbers are they.
