Mr Taillight quad headlight issue - Relay?

I got this mod a long time ago. Been working fine until the other day. I think the relay went bad

I can't read the # on the relay now.

Does anyone know what relay is the CORRECT model for this application?

Its just your standard 4 or 5 pin automotive relay
