Accessing heat shield retainer bolt heads??

My heat shield rusted around the support washers, and had fallen and was vibrating against the muffler. I wanted to remove the nuts and install an oversized washer then replace the nut. I PB blasted the few I could access with the vehicle rear up on jackstands, and after a short soak, I tried to remove the nut. I had assumed the bolt head must be captive somehow, but I can only get the rusted nut to turn so far before the head spins. From what I could see, the bolt comes through from the other side of the frame cross members, and is covered by the floorpan so I can't get a socket/wrench on the head to hold it while removing the nut.

For now I managed to rig up a quick fix by installing a 2nd nut and larger washer behind the first nut, but eventually I'll want to do it right. This must have happened to others, what was their solution? How do you access the bolt head, or for that matter remove the bolt if you want to replace it with a new one?
